Word Doc into a script

I have this MS Word document that is a form for someone to fill out as a supplemental. I want this to be turned into an online form and system for an admin to control. I also want it to be able to export the information into a PDF format and also XML for archiving. The admin will have the ability to login and view the form and the information where it came from, IP address with Ban IP capabilities and Print (PDF) and finalize the form by accepting or rejecting it and with a notes area as t…
